Immunization Record Information
Dear SJU Student,
Welcome to St. John's University! As part of your process prior to coming to campus, you are required to submit a record of your immunizations. If you are an international student, you must also have a tuberculosis screening. This information is due on or before June 15th for Fall semester admission or January 15th for Spring semester admission.
Access the form that you will need to complete in order to meet the requirements. Please follow these steps:
Read over what is required on the form.
Make an appointment with your medical provider. It is highly recommended that you meet with a provider to go over your immunization record, get the dates and doses of the immunizations you have had, get a provider signature stating you have the required immunizations, and discuss any other recommended vaccinations. The meningitis vaccine is strongly recommended for anyone planning to live in a residence hall on a college campus. Hepatitis A & B and the Varicella Vaccine (if you haven't had chicken pox) are also recommended.
If you are from a country other than the United States and currently residing there and are enrolled and planning on attending St. John's University, you are required to have a tuberculosis screening. The screening is set out in steps on the form for your medical provider to follow. It is important that all steps are followed and that your medical provider verifies that you are currently free of active tuberculosis if you had any positive test or abnormal x-ray.
Return the completed form (appropriate areas filled out or documentation attached and provider signatures, printed name, clinic name, address, & phone number included) to CentraCare Clinic St. John’s PO Box 7133 Collegeville MN 56321. Your completed form can also be faxed to (320) 200-3247 or emailed to email@example.com.
